Formula
eAG (mg/dL) = 28.7 × A1c (%) − 46.7; eAG (mmol/L) = 1.59 × A1c − 2.59. IFCC (mmol/mol) = (A1c% − 2.15) × 10.929
Clinical pearls
- Individual average glucose can differ substantially from eAG (95% CI at A1c 7% is roughly 123–185 mg/dL).
- A1c is unreliable with altered red-cell turnover: hemolysis, recent transfusion, iron deficiency, CKD, pregnancy, hemoglobinopathies.

FAQ
What average glucose is an A1c of 7%?
About 154 mg/dL (8.6 mmol/L) by the ADAG equation.
What A1c diagnoses diabetes?
An A1c of 6.5% or higher (confirmed) diagnoses diabetes; 5.7–6.4% is prediabetes.
