NEWS2 (National Early Warning Score 2)

NEWS2 (Royal College of Physicians, 2017) aggregates respiratory rate, SpO₂ (Scale 1, or Scale 2 for hypercapnic patients with target 88–92%), supplemental oxygen, systolic BP, pulse, consciousness (ACVPU) and temperature. 0–4 is low risk, a 3 in any single parameter needs urgent ward review, 5–6 is medium risk (urgent response) and ≥7 is high risk (emergency response).

  • Respiratory rate (/min)
  • SpO₂ (%)
  • SpO₂ scale: Scale 1 (standard); Scale 2 (hypercapnic respiratory failure, target 88–92%)
  • Air or oxygen: Room air (0); Supplemental oxygen (2)
  • Systolic blood pressure (mmHg or kPa)
  • Heart rate (/min)
  • Consciousness (ACVPU): Alert (0); New confusion, or responds to Voice/Pain, or Unresponsive (3)
  • Temperature (°C or °F)

Clinical pearls

  • Use SpO₂ Scale 2 only for patients with confirmed hypercapnic respiratory failure and a prescribed target of 88–92%, as decided by a clinician.
  • New confusion scores 3 (the "C" added to AVPU in NEWS2).
  • Not validated in children or pregnancy (use PEWS / MEOWS).

FAQ

What NEWS2 score triggers an emergency response?

A total of 7 or more. A score of 5–6 needs an urgent response, and a 3 in any single parameter needs urgent ward review.

When should SpO₂ Scale 2 be used?

For patients with hypercapnic respiratory failure (e.g. some COPD) who have a prescribed saturation target of 88–92%.
