EDACS (Emergency Department Assessment of Chest Pain Score)

EDACS scores possible cardiac chest pain from age, sex, risk factors and pain features. EDACS < 16 with no new ischemia on ECG and negative 0 h and 2 h troponins (EDACS-ADP) is low risk for 30-day MACE (sensitivity 99–100%) and suitable for early discharge.

  • Age (years)
  • Sex: Male; Female
  • Known CAD or ≥3 risk factors (counts only if age 18–50) (yes/no, +4)
  • Diaphoresis (yes/no, +3)
  • Pain radiates to arm, shoulder, neck or jaw (yes/no, +5)
  • Pain occurred or worsened with inspiration (yes/no, -4)
  • Pain reproduced by palpation (yes/no, -6)

Interpretation

ResultMeaning
< 16 + no new ischemia + negative 0 h/2 h troponinLow risk (EDACS-ADP): early discharge
≥ 16, or ischemic ECG, or positive troponinNot low risk

Formula

Age: 18–45 = 2, then +2 per 5-year band (46–50 = 4, 51–55 = 6 … 81–85 = 18, ≥86 = 20) · male +6 · age 18–50 with known CAD or ≥3 risk factors +4 · diaphoresis +3 · radiation to arm/shoulder/neck/jaw +5 · pain with inspiration −4 · pain reproduced by palpation −6

Clinical pearls

  • Risk factors: treated hypertension, dyslipidemia, diabetes, current smoking, family history of premature CAD. Known CAD = prior ACS, PCI or CABG.
  • The score alone is not the rule: low risk needs EDACS < 16 AND a non-ischemic ECG AND negative 0 h and 2 h troponins.
  • The CAD/risk-factor item only scores in patients aged 18–50; older patients already get it through age points.

FAQ

What EDACS score is low risk?

Below 16, provided the ECG shows no new ischemia and troponins at 0 and 2 hours are negative.

EDACS or HEART score?

Both identify low-risk chest pain; EDACS-ADP classified about half of patients as low risk with ~99–100% sensitivity for 30-day MACE in its validation.
