eGFR (CKD-EPI 2021, race-free)

Estimates glomerular filtration rate from serum creatinine, age and sex using the race-free 2021 CKD-EPI creatinine equation, the current recommended equation in North America. Normal is ≥90 mL/min/1.73 m².

  • Serum creatinine (mg/dL or µmol/L)
  • Age (years)
  • Sex: Male; Female
—

Interpretation

ResultMeaning
≥ 90G1
60–89G2
45–59G3a
30–44G3b
15–29G4
< 15G5

Formula

eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^age × 1.012 [if female]; κ = 0.7 (F) / 0.9 (M); α = −0.241 (F) / −0.302 (M)

Clinical pearls

  • Not valid in acute kidney injury (creatinine not at steady state).
  • For drug dosing, many labels still use Cockcroft-Gault creatinine clearance.

FAQ

Why was race removed from eGFR?

The 2021 CKD-EPI refit dropped the race coefficient after an NKF-ASN task force recommended race-free equations.
